Requesting an appointment
Submitting the new patient request form starts a request. It does not by itself establish care. A referral from a medical provider is required before Canyon Pain Center establishes a new pain-management patient, and our team will confirm the details with you.
Text messages and marketing emails
If you opt in, Canyon Pain Center sends text messages about your appointments and care, and separately — only where you have asked for them — occasional messages about the practice. Consent is never a condition of treatment or of any purchase. Message frequency varies, and message and data rates may apply. Reply STOP to cancel or HELP for help at any time. These are governed by our Text Message Terms, which form part of these terms. You must be at least 18 years old to opt a mobile number in; for a patient under 18, a parent or legal guardian gives consent and opts in their own number. Carriers are not liable for delayed or undelivered messages, and text messages are not guaranteed to arrive.
If you opt in separately, we also send occasional marketing emails about the practice, its services, and patient offers. Consent is never a condition of treatment or of any purchase. Every marketing email carries an unsubscribe link, and unsubscribing stops those emails without affecting your care or the emails we send you about your own appointments and billing. Text messages and marketing emails are separate programs with separate consent: opting in to one does not opt you in to the other, and stopping one does not stop the other. You must be at least 18 years old to opt an email address in.
Accuracy of information
We work to keep this site accurate and current. Clinical content is general and may not reflect the most recent developments in a given area, and pricing and scheduling information is subject to change. Where the site and our office differ, our office is correct.
Intellectual property
The content, layout, and design of this site, and the Canyon Pain Center name and logo, are owned by Canyon Pain Center and may not be reproduced without written permission.
