Scope of this policy
This policy covers information collected through canyonpaincenter.com. Protected health information created or received in the course of treatment is governed separately by our Notice of Privacy Practices.
Information we collect
Through this website we collect:
- Information you submit through the new patient request form, including your name, date of birth, contact details, and the clinical information the form asks for.
- Standard server information generated when any browser requests a page, such as IP address and browser type.
- Any information you choose to send us by email or telephone.
- Your mobile number and a record of your text-messaging consent, if you asked us to text you.
Cookies and tracking
This website sets no advertising or remarketing cookies and runs no tracking pixels. We set one first-party necessary cookie, cpc_consent, which records the choice you make in our cookie banner so it does not reappear on every page. It stores no personal, contact, or health information, expires after 180 days, and is never shared with anyone. You can delete it at any time through your browser settings. The pages always load the Google Fonts stylesheet, which serves the typefaces used across the site, and the Google Tag Manager container, which is the tool we use to manage measurement and which also proves to Google that we own this site. The container loads on every visit, but it starts in a denied state: it sets no analytics cookies and collects no measurement until you accept optional analytics. Once you accept, it enables Google’s own analytics cookies, which record how you move through the site and not who you are. The pages also always load Ahrefs Web Analytics, a measurement tool that counts page views. It sets no cookies and assigns you no identifier, so there is nothing about it to consent to and nothing it can use to recognise you on a later visit or on another website. It reports aggregate figures — how many times a page was viewed, and the referring site — and we never send it your name, contact details, or any health information.
Our cookie banner asks you to Accept or Decline optional analytics cookies, and neither answer is harder to give than the other. Declining is the default, and it is the state every visit begins in. You can change your answer whenever you like through the Cookie preferences link in the footer of every page. Analytics runs only for visitors who have accepted it: if you decline, or simply never answer, the Tag Manager container stays denied and no analytics cookies are ever set. Withdrawing an earlier acceptance takes effect immediately, stopping measurement and clearing the analytics cookies it had set. We never send your name, contact details, or any health information to it.
How we use your information
Information submitted through the request form is used to contact you about your request, to determine whether we can appropriately treat your condition, and to arrange care.
We do not sell personal information. We use information from the request form to respond to it and arrange care; we send practice news by text message or email only to patients who opt in to that channel on one of our forms, and you can withdraw either one at any time without affecting the other or your care.
Where form submissions go
New patient requests are transmitted over an encrypted connection to our patient management system, which acts as a business associate and is contractually required to safeguard your information and to use it only for the services it performs for us. Access is limited to Canyon Pain Center staff who need it to respond to your request.
Text messages and your mobile number
If you check the text-message box on one of our forms, or otherwise agree to be texted, we use your mobile number to send you appointment and care messages and — only if you also asked for them — occasional messages about the practice. Message frequency varies, and message and data rates may apply. You can reply STOP to any message to cancel, or HELP for help. The full details are in our Text Message Terms.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging originator opt-in data and consent are not shared with any third party, and are excluded from every category of information sharing described in this policy. Your mobile number and your consent are handled only by Canyon Pain Center and by the patient management system that sends the messages on our behalf, which is contractually barred from using them for its own purposes.
Consent to receive text messages is never a condition of treatment, of scheduling, or of any purchase. Declining changes nothing about the care you receive.
Marketing emails and your email address
If you check the email box on one of our forms, or sign our printed consent form, we use your email address to send you occasional practice news and offers — new services and providers, office updates, patient offers, and health information for our patients. A few emails a month at most. This is separate from text messages and separate from emails about your own care, appointments, and billing, which you receive because you are our patient and which continue whether or not you opt in to marketing.
Every marketing email carries an unsubscribe link at the bottom and our practice mailing address. Unsubscribing takes effect promptly and does not affect your care, your appointments, or any other email we send you. You can also unsubscribe by replying to the email, calling the office at (602) 603-2282, emailing info@canyonpaincenter.com, or telling any staff member.
We do not sell or rent your email address, and we do not share it with third parties for their own marketing. It is handled only by Canyon Pain Center and by the patient management system that sends the messages on our behalf, which is contractually barred from using it for its own purposes.

How long we keep it
Requests that become patient records are retained under the medical-record retention rules that apply to the practice. Requests that do not result in care are retained only as long as needed to answer your enquiry and to keep a record of our response.
Your choices
You may contact us at any time to ask what information we hold about you, to correct it, or to ask us to delete it where the law permits. Requests involving medical records follow the process set out in our Notice of Privacy Practices, and we respond within 30 days.
You may also ask us to stop contacting you at any time.
To stop text messages specifically, reply STOP to any message from us, or call the office at (602) 603-2282. Opting out of texts does not affect your care.
To stop marketing emails specifically, use the unsubscribe link at the bottom of any one of them, reply to it, or call the office. Texts and emails are separate programs: stopping one does not stop the other, and neither affects your care.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them through it.
Security
We use administrative, technical, and physical safeguards to protect the information we hold. No method of transmission over the internet is completely secure, so please do not send detailed medical information by unencrypted email. Use the request form or call the office instead.
Changes to this policy
We will post any changes on this page and update the effective date shown above.
