Dr. Sullivan is the Administrator and Director of our clinic. She is board-certified in Pain Management Procedures by the American Academy of Procedural Medicine and has completed advanced training in Internal Medicine, Preventive Medicine, and Occupational Medicine. Over the past 12 years, Dr. Sullivan has focused on pain management, workers' compensation, and legal medicine, offering compassionate care alongside expert medical evaluations. Her leadership is marked by clinical excellence, dedication to patient advocacy, and a strong commitment to the highest standards of healthcare delivery.

A referral from a medical provider is required to establish care as a new pain-management patient. Treatment recommendations are then made after an individual clinical evaluation of your history, exam findings, and goals.

Frequently asked questions

We are out of network with all insurance plans, and we bill select PPO plans directly on that basis. With any other plan your visit is self-pay, and you may request a superbill to submit to your insurer yourself; reimbursement is not guaranteed. Email info@canyonpaincenter.com to verify your plan before you schedule.
Once we have a referral from your provider, new patients are typically seen in about 7 days. Call us at (602) 603-2282 and our team will explain the process, pricing, and the earliest appropriate appointment.
Contact your previous provider's office and request a records release to Canyon Pain Center, or sign a release form at your first visit and we'll request them on your behalf.
