Kritika is a board-certified family nurse practitioner and the Lead Nurse Practitioner at our clinic. She leads day-to-day patient care, from new-patient evaluations through ongoing follow-up visits, and reviews your chart before your visit so you are not retelling your story from the beginning.

Clear pricing, explained upfront
We explain the appointment price and what to bring before you schedule, so there are no surprises. We bill select PPO insurance plans directly on an out-of-network basis; all other plans are self-pay.
Individualized, responsible care
A referral from a medical provider is required to establish care as a new pain-management patient. Treatment recommendations are then made after an individual clinical evaluation of your history, exam findings, and goals.

Frequently asked questions
We are out of network with all insurance plans, and we bill select PPO plans directly on that basis. With any other plan your visit is self-pay, and you may request a superbill to submit to your insurer yourself; reimbursement is not guaranteed. Email info@canyonpaincenter.com to verify your plan before you schedule.
Once we have a referral from your provider, new patients are typically seen in about 7 days. Call us at (602) 603-2282 and our team will explain the process, pricing, and the earliest appropriate appointment.
Contact your previous provider's office and request a records release to Canyon Pain Center, or sign a release form at your first visit and we'll request them on your behalf.
