How to get started

Ask your provider for a referral
A referral from a medical provider is required to establish care. Your physician or nurse practitioner sends a pain-management referral with your relevant records.

Request an evaluation
Call us or complete our evaluation request form. Tell us about your pain and what you want to change.

We explain everything first
Our team explains the appointment price, payment expectations, and what to bring so you can decide with confidence.

Your first visit
A thorough evaluation of your history, records, and goals, and you leave with a clear plan for what comes next.
Simple, transparent pricing.
We are out of network with all insurance plans. We bill select PPO plans directly on an out-of-network basis; with any other plan your visit is self-pay and you may request a superbill for possible reimbursement, which is not guaranteed.
