New Pain Management Patient Process

1. Get a referral from a medical provider for pain management.


2. Call and let us know you’re interested in becoming a new patient.


3. Bring in your medical records, or come in and fill out a medical record request form so that we can request your records directly from your previous provider.


4. Submit a new patient packet to ensure we receive your medical records.


5. Your new patient appointment will be scheduled once all is done.

Frequently Asked Questions for New Patients

Yes, you need a referral. We are currently unable to see or establish any patients without a referral.

We accept pain management referrals from medical providers, including doctors and nurse practitioners. If your medical provider deems that you are in need of pain management, they can write you a referral.

Once we receive and review your medical records, we will have an new patient appointment for you established the following week. 

To obtain past medical records, simply reach out to your previous medical providers directly. You can arrange to pick up your records in person. Alternatively, visit our office to sign a release to authorize our office to request the necessary records, in many cases, we can ask your medical provider to expedite the records so that you can get your appointment faster.
